We would like to understand the impact of the Coronavirus pandemic and subsequent restrictions on your child/young person’s short break. This online survey is for parents and carers of children and young people who receive an allocated short break.

The information we receive will help us identify the impact of the restricted short break service, review service quality and learn how we can improve.
